Its con cepts and tech niq ues have evolved from efforts to improve the safety of the complex

It is based on the ideas that accidents result from a nu mber of in teract ing causes with in a system, and that each cause and in teract ioncan be logicallyide ntified,evaluated, and con trolled.Through the logical applicati on of scie ntifi
