20111103015關(guān)鍵詞CFTRICP基因突變發(fā)病機(jī)制CFTR基因的結(jié)構(gòu)和功能1989年,CFTR基因首次被發(fā)現(xiàn),其位點(diǎn)在第七對(duì)常染色體的長(zhǎng)臂上(7q31),其相關(guān)基因位于染色體7p基因組DNA的250000對(duì)堿基上。
編碼產(chǎn)物是由1480個(gè)氨基酸組成的~種離子通道蛋白,即囊性纖維化穿膜傳導(dǎo)調(diào)節(jié)因子,它能調(diào)節(jié)和促進(jìn)上皮細(xì)胞膜電解質(zhì)離子的轉(zhuǎn)運(yùn)[z]。
在胰腺,TR分布于導(dǎo)管系統(tǒng)的腔面細(xì)胞膜上,為cAMP調(diào)控的Cr通道,使Cl一由胰腺導(dǎo)管細(xì)胞進(jìn)入腺泡腔或?qū)Ч芮?,繼而導(dǎo)管細(xì)胞腔面頂端原漿膜上發(fā)生電中性陰離子Cl一和HC0。

一由細(xì)胞內(nèi)轉(zhuǎn)移至腔內(nèi),并帶走水分,以利堿化胰液和稀釋胰液。
腺泡腔的pH值是酸性的酶原顆粒和導(dǎo)管細(xì)胞堿性分泌物動(dòng)態(tài)平衡的結(jié)果,腺泡腔正常的pH值可保證腺泡頂端原漿膜正常的酶原顆粒膜內(nèi)吞和稀釋聚集的胰酶[3.4]。
若CFTR功能不足,則腺泡腔的pH值下降,酶原顆粒膜內(nèi)吞障礙,使腺泡細(xì)胞變平,腺泡膨脹,內(nèi)充滿嗜酸性凝結(jié)物,直至外分泌功能封閉和含濃縮蛋白質(zhì)的胰導(dǎo)管阻塞[s]。
Andersen等【6]將正常CFTR基因轉(zhuǎn)染入CF細(xì)胞,可恢復(fù)正常的Cr轉(zhuǎn)運(yùn)。
CFTR基因突變CFTR基因突變可分成5類[8]:
①無義、框架移位和拼接突變,導(dǎo)致CFTR蛋白合成的截?cái)嗪腿狈?;②干擾正確的翻譯后過程,盡管蛋白本身可能保持正常的功能,但其中大多數(shù)不能到達(dá)導(dǎo)管細(xì)胞頂端原漿膜上;⑧突變致Cl一通道開放,表現(xiàn)為對(duì)cAMP刺激反應(yīng)的減弱,如G551D(甘氨酸在551位點(diǎn)突變?yōu)樘於彼?;④CFTR的Cl一通道離子選擇性和傳導(dǎo)性的改變;⑤干擾了正確的轉(zhuǎn)錄和翻譯,導(dǎo)致功能性CFTR水平降低。
