Partiesinvolved:

1.Trustor(hereinafterreferredtoas"Trustor")

2.Trustee(hereinafterreferredtoas"Trustee")

ObjectiveoftheTrust:

TheTrustorherebyestablishesthisTrustforthepurposeof(insertpurposeoftheTrust,e.g.,providingfinancialsupportfortheBeneficiary'seducation,managingtheTrustor'sassets,etc.).

Trustee:

TheTrusteeisappointedbytheTrustortomanageandadministertheTrustpropertyinaccordancewiththetermsandconditionsofthisTrustAgreement.TheTrusteeshallhavethefollowingdutiesandresponsibilities:

1.ManageandpreservetheTrustpropertyinaccordancewiththeTrustAgreement.

2.MakedistributionstotheBeneficiaryinaccordancewiththeTrustAgreement.

3.KeepaccuraterecordsofallTrusttransactionsandprovideannualreportstotheTrustorandBeneficiary.

4.ActinthebestinterestsoftheBeneficiaryatalltimes.

Beneficiary:

TheBeneficiaryistheindividualorentityentitledtoreceivedistributionsfromtheTrust.TheBeneficiaryshallhavethefollowingrights:

1.ReceivedistributionsfromtheTrustinaccordancewiththeTrustAgreement.

2.RequesttheTrusteetoprovideperiodicreportsonthestatusoftheTrustpropertyanddistributions.

3.ObjecttoanyactionstakenbytheTrusteethatarenotinaccordancewiththeTrustAgreement.

TrustProperty:

TheTrustpropertyshallconsistof(insertdescriptionofproperty,e.g.,cash,realestate,investments,etc.).TheTrusteeshallmanageandpreservetheTrustpropertyinaccordancewiththeTrustAgreement.

Distributions:

TheTrusteeshallmakedistributionstotheBeneficiaryinaccordancewiththeTrustAgreement.Thedistributionsshallbemade(insertfrequency,e.g.,annually,semi-annually,etc.)andshallbeinsuchamountsasspecifiedintheTrustAgreement.

DurationoftheTrust:

TheTrustshallcontinuefor(insertdurationoftheTrust,e.g.,theBeneficiary'sreachingacertainage,theTrustor'sdeath,etc.).UpontheexpirationoftheTrust,theremainingTrustpropertyshallbedistributedtotheBeneficiaryortransferredtoanotherTrust,asspecifiedintheTrustAgreement.

AmendmentandTermination:

ThisTrustAgreementmaybeamendedorterminatedbytheTrustor,uponwrittennoticetotheTrusteeandBeneficiary.AnyamendmentorterminationshallbeeffectiveuponthewrittenconsentoftheTrusteeandBeneficiary.

GoverningLaw:

ThisTrustAgreementshallbegovernedbyandconstruedinaccordancewiththelawsof(insertjurisdiction,e.g.,theUnitedStates,theUnitedKingdom,etc.).

EntireAgreement:

ThisTrustAgreementconstitutestheentireagreementbetweentheTrustor,Trustee,andBeneficiary,andsupersedesallprioragreements,representations,andunderstandings,whetherwrittenororal,relatingtothesubjectmatterofthisTrustAgreement.

INWITNESSWHEREOF,thepartieshaveexecutedthisTrustAgreementasofthedatefirstabovewritten.
