2、on.2. 范圍/Scope適用于全公司范圍內(nèi)的危險作業(yè)安全管理，包括動火作業(yè)、高處作業(yè)。This procedure ruled all hazardous operations including hot work; work at height;3. 定義/Definitions3.1 動火作業(yè)：進行焊接與切割作業(yè)及在易燃易爆場所使用噴燈、電鉆、砂輪等進行可能產(chǎn)生火焰、火花和赤熱表面的臨時性作業(yè)；Hot work: Any work involving welding, cutting, grinding or other similar operation producing spar

3、ks.3.2 高處作業(yè)：利用腳手架、升降平臺、便攜式木梯和金屬梯等登高工具從事超過基準面2米以上，存在墜落風(fēng)險的作業(yè)；Work at height:4. 程序/Procedure序號NO.危險作業(yè)內(nèi)容Hazardous operation4.1動火作業(yè) Hot work4.1.1職責(zé) Responsibility4.1.1.1申請人的職責(zé)（必須由我司項目負責(zé)人擔(dān)當(dāng)）：1.動火作業(yè)前負責(zé)動火作業(yè)申請，并遵守“一天一證一動火點”為原則；2.對外來施工單位的資格合法性審查，提醒外來施工單位帶齊動火所需要的相關(guān)證件、防護裝備；以備我司檢查；3.對動火作業(yè)現(xiàn)場進行確認，確?，F(xiàn)場符合動火要求；4.對動火人

4、員的勞動防護、動火設(shè)備的安全狀態(tài)負責(zé)；5.對動火人員進行培訓(xùn)，確保熟悉動火作業(yè)要求、消防器材使用方法和應(yīng)急處理流程（必要時，聯(lián)系EHS人員助指導(dǎo)）；6.每天上、下午各不少于一次不定時對動火現(xiàn)場進行巡視，發(fā)現(xiàn)異常立即予以糾正；7.所有供應(yīng)商外服人員、外來施工人員等進行的焊接、氣割、烘烤等明火作業(yè)均需要提前申請；Responsibility of applicant (The applicant should be the project leader):1. A hot work permit needs filled out before hot work is performed EXCEP

5、T if it is normal production operation. For example welding area. One permit is valid for one job site and one day.2. The fire operator must be trained specially, and get the license, the project leader is responsible for checking out the license of fire operator, informing constructor to carrying t

6、he license and wear proper personal protective equipment. 3. Check out fire-proof measure including condition of site, preventive measures to make sure its safe to perform hot work.4. Check out fire operators PPE and tools.5. Train fire operators about safety requirements for hot work, and how to us

7、e fire facilities and evacuation.(If necessary, contact EHS)6. Check out the job site at least one time irregular in morning and afternoon. Shut down and make corrective action if there are any unsafe conditions or actions regarding cutting, welding and/ or heating operations.7. A hot work permit is

8、 required prior to start any hot work.4.1.1.2審核人的職責(zé)（動火區(qū)域主管，及EHS部門擔(dān)當(dāng)）：1.對動火作業(yè)現(xiàn)場進行確認，確?，F(xiàn)場符合動火要求；Supervisor of job site and EHS department sign and approve the permit:1. Go to job site and check out the fire-proof measures before approving the operation, ensure that its safe to perform hot work.4.1.1.3

9、動火人職責(zé)1.查驗動火作業(yè)申請單審批情況，無動火作業(yè)申請單、申請單上相關(guān)流程未審批完畢的應(yīng)拒絕進行動火作業(yè)；2.逐項確認相關(guān)安全措施的落實情況；3.確認動火地點和時間，是否和動火作業(yè)申請單一致；4.發(fā)現(xiàn)不具備安全條件時不得進行動火作業(yè)；5.隨身攜帶動火作業(yè)申請單，以備檢查；6.所有金屬焊接、氣割作業(yè)人員必須有“金屬焊接切割作業(yè)操作證”，并熟悉動火要求及消防器材使用方法；7.動火人員必須佩戴焊工專用絕緣手套、絕緣鞋及其它防護用品；8.在動火作業(yè)完成后，清理現(xiàn)場，清除殘火，確認無遺留火種后方可離開現(xiàn)場。Responsibility of fire operator:1. Check out hot

10、 work got approval. If there is no hot work permit, fire operator has right to refuse fire operation.2. Check out fire-proof measure including condition of site, preventive measures to make sure its safe to perform hot work.3. Check out job site is according with hot work permit.4. Shut down fire op

11、eration if there are any unsafe conditions.5. Post up hot work permit in job site6. Fire operator must be trained specially and get the license.7. Wear proper personal protective equipment such as gloves, safety shoes and face shield.8. Clean the job site; make sure no fire in job site when finishin

12、g fire operation.4.1.1.4監(jiān)火人職責(zé)1.堅守崗位，不準脫崗若暫時離開需找適合人員替崗，并盡快返回；動火期間，不準兼做其它工作； 2.負責(zé)現(xiàn)場動火前的準備工作并監(jiān)護與檢查，發(fā)現(xiàn)異常情況應(yīng)立即通知動火人停止動火作業(yè)，及時聯(lián)系有關(guān)人員采取措施； 3.當(dāng)發(fā)現(xiàn)動火人違章作業(yè)時應(yīng)立即制止；4.在動火作業(yè)完成后，應(yīng)會同有關(guān)人員清理現(xiàn)場，清除殘火，確認無遺留火種后方可離開現(xiàn)場。5. 監(jiān)火人需在動火作業(yè)完成1小時及3小時內(nèi)，返回現(xiàn)場檢查確認無遺留火種。Responsibility of fire watcher:1. Fire watcher must constantly roam th

Fire watch has the authority and responsibility to shut down the hot work when he is aware of any unsafe conditions or actions regarding cutting, welding and/ or heating operations and take corrective

14、 action 3. When there are some illegal operations, shut down hot work.4. Clean the job site together with fire operator and make sure there is no fire risk5. The fire watcher shall be presented for at 1 hour and 3 hours after hot work is done.4.1.2動火作業(yè)安全要求 Safety requirement for hot work:4.1.2.11.所有

15、動火申請由本公司項目負責(zé)人負責(zé)，以“一天一證一動火點”為原則，負責(zé)現(xiàn)場安全檢查和應(yīng)急處理，熟悉消防器材使用方法；2.動火人、監(jiān)火人都必須熟悉動火要求，并承擔(dān)現(xiàn)場監(jiān)督、防火管理責(zé)任。3.五級以上風(fēng)力或雨雪天氣，禁止室外動火作業(yè)；4.所有金屬焊接、切割作業(yè)人員必須有“金屬焊接切割作業(yè)操作證”，為防止觸電事故的發(fā)生，動火人員必須佩戴焊工專用絕緣手套、絕緣鞋及其它防護用品，包括外服人員和外來施工人員，并保證焊機絕緣層無破損；5.動火作業(yè)前必須將周圍易燃易爆物品移至安全位置。一般情況下，在施工區(qū)10m范圍內(nèi)，無可燃物，易燃液體，易燃氣體，可燃粉塵等均清理干凈。如仍不能滿足安全要求，若半徑10m范圍內(nèi)的易

16、燃、易爆品不能撤離，用防火布或濕木板隔離 ；（特別說明：靠近叉車電瓶區(qū)域作業(yè)時，必須將電瓶拆除或者切斷電瓶電源，并進行充分通風(fēng)，保證作業(yè)區(qū)域內(nèi)無燃爆氣體后，方可動火，防止氫氣爆炸）6.動火期間距離動火區(qū)域10米內(nèi)不得排放各類可燃氣體，可燃液體；在動火點10米范圍內(nèi)及用火點下方不得同時進行可燃溶劑清洗、噴漆或拋丸等作業(yè)。7.所有氣瓶嚴禁靠近火源、熱源和電氣設(shè)備,與明火距離不少于5米；氧氣瓶和乙炔氣瓶距離不少于5米。乙炔瓶禁止使用其它氣瓶進行替代，并保證有止回火閥。所有氣瓶必須無破損，壓力正常；8.動火現(xiàn)場配置兩只以上滅火器，必要時將消防水帶鋪設(shè)到作業(yè)現(xiàn)場。無火警時禁止使用，否則照價賠償，使用完成

17、，及時放回指定位置；9.動火結(jié)束后，動火人員和監(jiān)火人須共同進行確認，保證現(xiàn)場無火警后，方可離開作業(yè)現(xiàn)場。動火作業(yè)申請單應(yīng)交回EHS留存。1. The project leader will fill out hot work permit prior to start hot work; Check out fire-proof measure including condition of site, preventive measures to make sure its safe to perform hot work; knowing how to deal with emergency

18、 is basic requirement.2. Fire watcher and operator must be familiar with safety requirement for hot work.3. Hot work in open air is forbidden when there is bad weather such as rainstorm, and heavy wind above five grades that influence the safety of work.4. Fire operator must be trained specially, an

19、d gets the license. They should wear proper personal protective equipment to ensure safety operation. Any damaged or frayed cord in welding machine should remove from service. The constructors are required to follow this requirement.5. Exposed flammable/combustible materials and equipment with a 10m

20、 radium must remove or be protected by fire-proof covers or fire blankets before hot work begins.(Remark: Shut off the power and keep good ventilation while fire operation being done near charging area, it will prevent gas explosion)6. All combustible dust ,vapor, gas of liquids must be kept 10m awa

21、y or properly covered with fire-proof curtains, fire blankets. It means weld/cut within 10m was forbidden while a paint booth or shot blaster was running.7. Oxygen cylinder should keep at least 5m away from acetylene cylinder. Meanwhile gas cylinder should keep 5m away from ignition source and in go

22、od condition.8. Two or more fire extinguishers must be kept in job site.9. Clean the job site when finished fire operation and make sure there is no fire risk. The fire watcher shall be presented for at 1 hour and 3 hours after hot work is done. EHS department will archive hot work permit.4.2高空作業(yè) Wo

23、rk at height4.1.1職責(zé) Responsibility4.1.1.1申請人的職責(zé)（必須由我司項目負責(zé)人擔(dān)當(dāng)）：1.高處作業(yè)前負責(zé)高處作業(yè)申請，并遵守“一天一證一作業(yè)點”的原則；2. 對外來施工單位的資格合法性審查，提醒外來施工單位帶齊高處作業(yè)所需要的相關(guān)證件、防護裝備、簽訂施工商廠內(nèi)作業(yè)安全協(xié)議；以備我司檢查；3.對高處作業(yè)人員及監(jiān)護人員進行培訓(xùn)，確保熟悉高處作業(yè)要求；4.每天上、下午不少于一次對作業(yè)現(xiàn)場進行巡視；5.要求作業(yè)方作業(yè)結(jié)束后，將現(xiàn)場整理整頓。Responsibility of applicant (The applicant should be the projec

Clean job site when finished to ensure job site orderly and clean.4.2.1.2會簽人的職責(zé)（使用部門主管擔(dān)當(dāng)）：1.對作業(yè)現(xiàn)場進行確認，確保現(xiàn)場符合高處作業(yè)要求；2.協(xié)調(diào)并避免施工與生產(chǎn)之間的交叉作業(yè)；3.確認高處作業(yè)下方3米范圍內(nèi)無其他作業(yè)。Supervisor of job site is responsible to:1. Check out the precautions before approving the operation; ensure that its safe to perfor

27、m work.2. Coordinate to prevent daily work conflict with work at height3. Check out there is no work with a 3 meters radium.4.2.1.3審核人的職責(zé)（安全部門擔(dān)當(dāng)）； 1.高處作業(yè)前對作業(yè)現(xiàn)場進行確認，確?，F(xiàn)場符合作業(yè)要求；2.檢查作業(yè)人員高處作業(yè)申請單手續(xù)申請是否齊全、作業(yè)人員防護是否配齊；施工方針對所有高處作業(yè)的管理要求是否了解；3.審核人員每天對施工現(xiàn)場上下午各不少于1次巡查，發(fā)現(xiàn)異常立即予以糾正，并記錄。Responsibility of auditor (fa

29、 least one time, if there is any unsafe condition, shut down the operation and take corrective actions.4.2.1.4作業(yè)人員的職責(zé)1.針對身體狀況負責(zé)，有高處作業(yè)禁忌的癥狀（高血壓、心臟病、貧血、癲癇等）視力不佳、疲勞過度等不適宜進行高處作業(yè)的情況，嚴禁高處作業(yè)；2.查驗高處作業(yè)申請單的審批情況，無高處作業(yè)申請單、申請單上相關(guān)流程未審批完畢的應(yīng)禁止作業(yè)；3.掌握高處作業(yè)的安全要求，并逐項確認相關(guān)安全措施的落實情況；4.配齊防護設(shè)備，對于危險性過高且無法進行安全保障的應(yīng)禁止作業(yè)；5.高于5米的

30、高處作業(yè)（3個腳手架高度）必須持證上崗；6.確認高處作業(yè)地點和時間，是否和高處作業(yè)申請單一致。Responsibility of operators:1. A regular physical for operators engaged in work at height is required: people who has occupational contraindication (such as hypertension, cardiopathy, anemia, epilepsy, and mental illness), oldness, weakness, overtired,

31、poor eyesight or other improper people are not allowed to engage in work at height.2. Check out the work has got approved. Operator has right to refuse perform the work if there is no work permit. 3. Understand safety requirement for work at height, and check out preventive measures one by one accor

32、ding to work permit.4. Wear proper personal protective equipment, operator has right to refuse the work with high risk.5. If operator is working above 5m, he shall receive special technology training, pass relevant test and gain authorization.6. Check out the job site and work time is according with

33、 work permit.4.2.1.5監(jiān)護人員的職責(zé)（可以為施工方的人員）1.高處作業(yè)監(jiān)護人必須全程監(jiān)護，堅守崗位；對使用靠墻梯進行高處作業(yè)的，監(jiān)護人員必須扶住梯梁2.監(jiān)護高處作業(yè)人員正確履行各項安全職責(zé)、落實各項安全措施；3.隨身攜帶高處作業(yè)申請單，以備檢查。Responsibility of watch master:1. Watch master must constantly roam the work point that he is assigned for. When using ladders or stepladders, watch master must assist

34、to hold it.4.2.3作業(yè)安全要求 Safety requirement4.2.3.1針對所有高處作業(yè)的管理要求 Safety management requirement1.高處作業(yè)者無職業(yè)禁忌的癥（高血壓、心臟病、貧血、癲癇等）視力不佳、疲勞過度等不適宜進行高處作業(yè)；2.高于5米的高處作業(yè)（3個腳手架高度）必須持證上崗； 3.高處作業(yè)必須佩戴好安全帽、安全帶；安全帽必須扣緊下顎帶，鎖緊后定鎖；安全帶必須系在作業(yè)處上方的牢固構(gòu)件上或?qū)閽彀踩珟У匿摷芑蜾摻z繩上，不得系掛在移動或不牢固的物件上；不得系掛在尖銳棱角部位，不得底掛高用；4.高處作業(yè)監(jiān)護人必須全程監(jiān)護，堅守崗位；對使用靠墻

35、梯進行高處作業(yè)的，監(jiān)護人員必須扶住梯梁；5.高處作業(yè)所使用的工具、材料、零件必須裝入工具袋；6.高處作業(yè)時，工具、材料、零件嚴禁隨意亂置或向下丟棄，使用完畢后，必須及時放入工具袋中；7.雨天、雪天、6級以上強風(fēng)、濃霧等惡劣氣候，嚴禁室外登高作業(yè)；8.夜間登高作業(yè)，必須有足夠的照明；Safety requirement:1.
