1、PAGE9叉車安全操作程序1.PURPOSE目的Thisprocedureprovidestheminimumrequirementsforthesafeoperationofforklifttrucks.Itenhancesthesafetyofallpersonnelinvolvedintheoperationofforklifttrucks,andalsootherswhomaybeworkingincloseproximitytoforklifttrucks.本程序提供最基本的叉車安全操作要求。旨在提高叉車操作人員以及叉車附近其他工作人員的安全性。2.SCOPE范圍Thisproced

2、ureappliestoallforklifttruckoperationsontheterminalregardlessoftheownershipoftheforklifttrucks.本程序適用于庫區(qū)范圍內(nèi)所有叉車操作（無論何方所有）。3.DEFINITION定義3.1.QualifiedForkliftOperatorisanoperatorwhohasundergonetherequiredtrainingonforkliftoperationsandhasthelicenseissuedbytheinterrelatedauthority.合格叉車操作人員指經(jīng)相關(guān)叉車操作培訓(xùn)并擁有

3、相關(guān)管理部門頒發(fā)的證書的操作人員。3.2.FlammableLiquidsrefertoproductswithflashpointbelow37.8OC.易燃液體指閃點低于37.8OC的產(chǎn)品。4.RESPONSIBILITIES職責(zé)4.1.Itistheresponsibilityoftheforkliftoperatortoconformtothestipulatedsafetyrequirementsonuseofforklift.叉車操作人員有責(zé)任遵守與叉車使用相關(guān)的安全要求。4.2.Itistheresponsibilityofthelinemanagers/supervisorst

4、oensurethatonlytrainedoperatorsareauthorizedtooperateaforklift.直接主管負(fù)責(zé)確保：只有經(jīng)過培訓(xùn)的有資質(zhì)的人員才能操作叉車。Theyshallalsoensurethattheforkliftsuppliedbycontractorareperiodicallymaintainedandserviced.Iftheforkliftsaretobeusedforhandlingofflammableliquidindrums,theyshouldbeinstalledwithsparkarrestingdevicessuchasarre

5、stors,flamearrestor,air-startdevice,etc.Inshort,theseforkliftsshallbedesignedforuseinClass1Division2(orZone2)ClassifiedHazardousAreas(refertoSHE-SWP-16).并應(yīng)確保承包商提供的叉車接受定期維修和保養(yǎng)。若叉車用于桶裝易燃液體搬運，應(yīng)安裝阻火設(shè)備如阻火器、火焰防止器、氣動裝置等。簡而言之，叉車應(yīng)適用于危險區(qū)域分類1級2分區(qū)（參見SHE-SWP-16）。4.3.Contractorwhosuppliestheforkliftsisresponsible

6、tomakethenecessaryarrangementsforanyforkliftrepairorperiodicservicingandinspectionbythemanufacturersoastoupkeepthesafeoperabilityoftheforklifttrucks.提供叉車的承包商負(fù)責(zé)就制造廠家叉車維修或定期保養(yǎng)、檢查服務(wù)進行必要的安排，以維持叉車的安全可操作性。5.SAFETYREQUIREMENTS安全要求5.1.Allforklifttrucksmustonlybeoperatedbyqualifiedoperators.只有有資質(zhì)的人員才能進行叉車操作。

7、5.2.ForklifttrucksshouldbesuitableforuseinClass1Division2(orZone2)ClassifiedHazardousAreasiftheyareusedforhandlingofflammableliquidsindrums.Themaximumexhaustsurfacetemperatureshouldbeabout200OC(IECT-Code).若用于桶裝易燃液體搬運，則叉車應(yīng)適合在危險區(qū)分類1級2分區(qū)（參見SHE-SWP-16）中使用。最大表面釋放溫度應(yīng)為200OC（IECT-編碼）。5.3.Alltrafficrulesandw

8、arningsignsontheterminalaretobestrictlyobeyed.嚴(yán)格遵守庫區(qū)所有交通規(guī)則和警示標(biāo)記。5.4.Ifthereisaneedtooperateaforkliftinhazardousareas,otherthantheroadwaysandDrumYard/Warehouse,theappropriateHotWorkPermitmustbeobtainedfromCCR.若有必要在危險區(qū)內(nèi)而非道路上或桶堆放場/倉庫內(nèi)進行叉車操作，則必須從中控室取得熱作業(yè)許可證。5.5.Loadlimitsoftheforklifttrucksmustbestencil

9、edonthemastofeachtruckwhereitisclearlyandreadilyvisibletotheoperator.叉車荷載極限必須印在叉車升降架上方以便操作人員清楚看到的位置。5.6.Beforeoperatingtheforklifttruckatthebeginningoftheday,theoperatormustinspecttheforklifttrucktoverifythatitisinagoodandsafeworkingconditions.在每天開始叉車操作前，操作人員必須對叉車進行檢查，以確定其處于良好、安全的工作狀態(tài)。5.7.Theoperato

10、rmustbealertandcautiouswhenoperatingtheforklifttruck.Hemustwatchforothervehicles,equipment,obstacles,roadsurfaceconditionsandpedestrians.操作人員在叉車操作時必須保持警覺和謹(jǐn)慎。操作人員必須注意其它車輛、設(shè)備、障礙、路面狀況和行人。5.8.Nopassengerisallowedtorideontheforklifttruck.叉車不許搭載乘客。5.9.Theoperatorshallfaceinthedirectionoftravel.操作人員應(yīng)面對行駛方向

11、。5.10.Theoperatorshallkeepthemasttiltedbackwithforks150to300mmabovethegroundwhiletravelingloadedorunloaded.行駛中無論有無負(fù)重，操作人員應(yīng)將前叉后傾，車叉距離地面150至300mm。5.11.Theoperatorshallcheckforadequateoverheadandlateralclearancebeforeenteringandduringsubsequentmaneuveringinanybuilding,plantfacilityorcongestedarea.在進入任何

12、建筑、裝置設(shè)施或密集區(qū)域之前及之后操作中，操作人員應(yīng)檢查上方及側(cè)面距離是否足夠。5.12.Theoperatorshallsoundthehornatjunctions,cornersandinareaswherevisionisbeingobstructed.在交叉口、轉(zhuǎn)彎處以及視線受阻區(qū)域內(nèi)，操作人員應(yīng)鳴喇叭。5.13.Theoperatorshalldriveslowlyandcautiouslyinthefollowingcircumstances.在以下情況下，操作人員應(yīng)緩慢、謹(jǐn)慎駕駛：whenmakingaturn轉(zhuǎn)彎時whenreversing倒車時drivingoverahum

13、p凹凸不平地面行駛時drivingupordownaramp上下坡道時drivingonawet,slipperyorunevensurface濕、滑、不平地面行駛時drivinginnarrowroads狹窄路面行駛時drivinginhighnoiseareasorwherevisionisobstructed高噪音區(qū)或視線受阻區(qū)域內(nèi)行駛時drivingincloseproximitytopotentiallyhazardousarea危險區(qū)附近行駛時5.14.Theoperatorshalltravelinthereversemodeiftheloadbeingcarriedobstru

14、ctstheforwardview.若所載重物阻礙前方視線，操作人員應(yīng)倒行。5.15.Theoperatormustalwaysensurethatloadlimitsarenotexceeded.Heshallnotcarryaloadifitisnotproperlysecuredorifitextendsabovethecarriagebackrestunlessnopartoftheloadcanpossiblyslidebacktowardstheoperator.操作人員必須確保不超出荷載極限。若重物未適當(dāng)固定或伸出貨物安全架，操作人員不得承運，重物任一部分沒有向后滑向操作人員的可

15、能時除外。5.16.Theoperatorshallavoidmakingsuddenstops/startandalso,runningoverlooseobjectsandholeswhichmaybepresentinthepathoftravel.操作人員應(yīng)避免急停急起，并避免在有松軟物體或有坑的路面上行車。5.17.Forklifttruckshallnotbeusedasaworkingplatformforpeople.Itshallnotbeusedforanypurposeotherthanthatforwhichitwasdesigned.叉車不得用作人員工作平臺。除設(shè)計目

16、的外，不得將叉車用于其它用途。5.18.Nopersonnelisallowedtostandorwalkundertheelevatedportionoftheforklifttruckwhetherloadedornot.無論有無重物，任何人不得在叉車抬高部分下站立或行走。5.19.Collisions,damagesornearmissesshallbereportedtotheShiftleaderimmediately.若有碰撞、損害或未遂事故，應(yīng)立即報告當(dāng)班領(lǐng)班。5.20.Noforkliftoperationisallowedonflatbedtrailersorlorries,

17、exceptforstuffingofcontainers.叉車不得在平板拖車或卡車上進行操作，集裝箱裝貨時除外。5.21.Incaseofanincidentinvolvingspillageofflammableliquid,theoperatorshouldattempttoswitch-offtheforkliftengineandnotifyCCR.若易燃液體泄漏引發(fā)事故，操作人員應(yīng)盡量關(guān)閉叉車發(fā)動機，并通知中控室。5.22.Prolongoperationofengine-drivenforklifttrucksinpoorlyventilatedareasmustbeavoide

18、d.必須避免內(nèi)燃機叉車在通風(fēng)不良區(qū)域內(nèi)長時間作業(yè)。5.23.Wheneverleftunattended,theoperatorshallparktheforklifttruckinalocationthatdoesnotobstructsafetyorfireprotectionequipmentandroadways.Thefollowingactionsneedtobedone.離開叉車前，司機應(yīng)將叉車停在對安全或消防設(shè)備與通道無妨礙的位置。需采取以下措施：a.theforksloweredandtiltedforwardsothattheforktipstouchtheground.降

19、下前叉，以便前叉能接觸地面。b.thefunctionalcontrolinneutralmode控制開關(guān)在空檔狀態(tài)。c.theparkingbrakeengaged使用停車制動。d.theignitionorkeyswitchturnedoff關(guān)閉點火或鑰匙開關(guān)。5.24.Whileoperatingtheforklifttruck,theoperatormustputonsafetyshoes.Hearingprotectiondeviceneedstobeworntoo.進行叉車操作時，操作人員必須穿上安全鞋，并使用必要的聽力保護用品。5.25.Hosesmaybecarriedbyaforklifttruckiftheyareproperlyenclosedwithinapalletboxwhichissecuredtotheforklifttruck.Carriageofhoseshangingfromtheforksisextremelyunsafeandisnotallow
