執(zhí)行安全生產(chǎn)的文件Documentsregardingsafetyproductiontobefollowed●國家政府頒布的安全生產(chǎn)規(guī)定的文件。Documentsissuedbythestateregardingsafetyproduction;●上海石化股份有限公司有關(guān)安全生產(chǎn)的規(guī)定。RulesmadebyShanghaiPetrochemicalCompanyLimitedregardingsafetyproduction;●公司有關(guān)安全生產(chǎn)的規(guī)定和文件。Rulesandregulationsmadebyourcompanyregardingsafetyproduction.5.7.2開工前安全交底DetailedsafetypresentationbeforeKick-offoftheproject在項目開工前，項目所有人員應(yīng)接受業(yè)主安全部門的安全交底，以了解施工環(huán)境情況和安全注意事項。Everyoneshallattendadetailedsafetypresentationgivenbythesafetydepartmentoftheownerpriortothestartoftheprojectinordertohaveagoodunderstandingofsafetyissuesandworkingconditionsattheconstructionsite.在每項新工作開工前，專職安全員應(yīng)向施工人員進(jìn)行安全交底，使每個施工人員明白潛在危險的工作及所要采取的安全措施。Thesafetysupervisorshallgiveconstructionworkersadescriptionregardingsafetybeforeeachnewworktoenableeachworkertobeawareofpotentialrisksatjobsiteaswellasthepreventionmeasurestobetakenaccordingly.保留交底的書面記錄。Safetypresentationanddescriptionshallbefiledinwrittenform.5.7.3現(xiàn)場安全檢查SitesafetyInspection●建立系統(tǒng)性的現(xiàn)場安全檢查制度，由安全員每日巡檢并重點監(jiān)控主要施工部位，正式的安全檢查每周至少進(jìn)行一次。Tosetupasystematicsitesafetychecksystem;thesafetysupervisorshallcheckandcontrolmainconstructionplacedaily;regularsafetycheckingshallbeperformedonceaweekminimum.●每一個現(xiàn)場人員的衛(wèi)生和安全行為將被嚴(yán)密監(jiān)督，使其規(guī)范化。Aclosesupervisiononeverysiteworkingpersonforhis(her)behaviorwithrespecttohealthandsafetyistobecarriedout.●檢查出來的問題，立即進(jìn)行整改，以消除危險，保證施工安全。Anyproblemthathasbeenfoundoutwillbeimmediatelycorrectedtoeliminatetoensurethesafetyofconstruction.5.7.4作業(yè)票制度OperationPermitssystem●現(xiàn)場項目部應(yīng)遵守業(yè)主有關(guān)作業(yè)票制度，以防止安全事故的發(fā)生。Siteprojectdepartmentshallfollowanoperationpermitssystemimplementedbytheownertopreventincurringsafetyproblems.●施工作業(yè)證含以下類型：Constructionoperationpermitsincludethefollowing:a）動火證：焊接、切割、及任何明火工作，均需持有允許動火證，動火證必須明確動火地點、時間范圍，并指定動火人。HotWorkPermit:welding,cuttingandanyotherworkneedsafireshallhaveaWorkingwithFiringPermit;suchWorkingwithFiringPermitshallindicatethepersonwhomistouseafire,thelocationwherethefiringworkisgoingtotakeplaceandthetimeframe;b）動電證：任何電氣作業(yè)人員均需具有資格證書，須接線動電，應(yīng)辦理動電許可證。WorkingrequestingElectricityPermit:anyelectricianshallhaveaelectricianqualificationlicense.HeshallapplyforaWorkingrequestingElectricityPermitwhenapowercablewiringand/orpowerenergizingactionistobemade;c）探傷作業(yè)證：使用X射線和γ射線進(jìn)行探傷檢測時，必須獲得探傷作業(yè)證，并采取適當(dāng)防護(hù)措施。Non-destructiveTestingPermit:anypersonwhowilldonon-destructivetestingshallgetalicense.HeshallbearmedwithprotectivefacilitywhendoingtestswithX-rayorγ-raymachines.d）車輛通行證：所有車輛進(jìn)入施工現(xiàn)場，均應(yīng)按規(guī)定辦理車輛通行證，戴好防火罩。VehicleTransportationPermit:anyvehicleneedstogointoconstructionsiteshallapplyforVehicleTransportationPermitandtakeafireprotectivehoodwhenitisreadytogointothesite.5.7.5臨時用電的規(guī)定RegulationsaboutTemporaryPowerUsing●施工現(xiàn)場的臨時用電的電源箱應(yīng)合理布置。電源電纜敷設(shè)安全可靠。為防止電纜損壞，應(yīng)架空或者有效敷設(shè)。配電箱的設(shè)置安全醒目，使用方便。Eachtemporarypowersourcecabinetatsiteshallbereasonablyallocated.Powercablesshallbesafelyandreliablylocatedhighincabletrayorinothereffectivemethodtoavoidgettingdamaged.Suchpowersourcecabinet,whichshallbeusedeasily,shallbeinstalledinasafepositionwhereiseasilyseenbypeopleatthesite●施工配電箱、盤都應(yīng)安裝斷電保護(hù)器，有良好的接地裝置。Allswitchboxesorpanelsshallbeequippedwithcircuitbreakerandreliableearthingdevice;●所有的施工用電氣設(shè)備，應(yīng)定期進(jìn)行檢查。發(fā)現(xiàn)問題及時整修或調(diào)換。電氣設(shè)備使用完后，一定要關(guān)掉電源。Allelectricfacilitiesforconstructionmustbeperiodicallychecked.Anyproblemsfoundwiththefacilityshallgetrepairedorreplaced;allelectricfacilitiesmustbeswitchedoffwhenworkisfinished;●配電箱盤開關(guān)的外殼和閘刀開關(guān)均應(yīng)保持完整無損。熔絲（片）應(yīng)按設(shè)備或線路容量確定。Theserviceofthepowersourcecabinet,panelandswitchshallbekeptcompleteandundamaged.Fusestobeusedshallbechoseninaccordancewiththespecificationoftheequipmentorcapacityoftheline;●非電工嚴(yán)禁擅自裝接用電器具,拉接電線。Anypersonsotherthanelectricianarenotpermittedtoinstallelectricalusersortowireapowercable.5.7.6施工腳手架搭設(shè)使用規(guī)定Rulesforinstallationofscaffoldings●腳手架的搭設(shè)應(yīng)填寫申報委托單及審批制度，手續(xù)需齊全。Scaffoldinginstallingshallmakeapplicationform.Approvalpermitshallbeavailablebeforehand;●施工腳手架建立腳手架搭設(shè)分階段驗收制度和專人維護(hù)保養(yǎng)。Sectionalcheckingandacceptancesystemshallbesetupforscaffoldinginstallingandmaintenanceservicebydesignatedpersonsystemshallalsobeestablishedforscaffoldingwhenitisbuiltup;●腳手架的材料及搭設(shè)，嚴(yán)格按照規(guī)程的要求搭設(shè)。具體要求見《腳手架安全搭設(shè)技術(shù)規(guī)定》。Materialschosenandinstallingofscaffoldingshallbemadestrictlyaccordingtorelevantregulation.(Thedetailscanbeseenin<SpecificationsforSafeInstallationofScaffoldings>).5.7.7施工機械使用規(guī)定Rulesforusingconstructionmachines●現(xiàn)場需用的各類機械必須經(jīng)過維護(hù)、保養(yǎng)。確認(rèn)機械的性能完好，方能使用。Alltypesofmachinestobeusedinconstructionsitemusthavebeenwellmaintained.Anymachinemustbecheckedifitstillhasagoodperformancebeforeusingit.●現(xiàn)場使用的各類機械應(yīng)定機定人，特殊施工的機械的操作人員，應(yīng)經(jīng)培訓(xùn)考試合格，持有安全操作證。Adesignatedpersonmustbeappointedforamachinetobeuseatsiteforitsoperationandmaintenance.Anoperatorforrunningaspecialmachinemustgettrained,passanexaminationandhaveacquiredaOperationLicense;●施工機械應(yīng)有良好的安全保護(hù)罩殼，并接地良好。Machineshallbeequippedwithprotectivehoodandwellgrounded;●各類機械的操作人員應(yīng)了解施工機械的使用性能和操作要求。使用應(yīng)檢查是否完好無損，操作應(yīng)遵守操作規(guī)程，發(fā)現(xiàn)故障應(yīng)立即檢修，嚴(yán)禁超負(fù)荷使用。Operatorsoperatingmachinesshallhaveagoodunderstandingofperformanceandoperatingrequirementsofthemachineshetakeschargeof.Heshallcheckitandbesurethatitisingoodconditionpriortorunningit.Operationproceduresshallbefollowedduringoperation.Machinesmustbestoppedwheneveramalfunctionisdetectedandgetitrepairedafterwards.Runningwithoverloadingisstrictlyprohibited.5.7.8起重機械（吊車）的使用規(guī)定Rulesforhoisting(cranes)machines●使用大型起重機、吊車必須按施工方案操作，明確其所使用的起重機能量和性能。Therunningofanytypeoflargesizehoistingmachine,cranesshallfollowaWorkingProgrampreparedforit.Thecapacityandperformanceofthehoistingmachinetobeusedmustbemadewellunderstood;●使用時應(yīng)有專人負(fù)責(zé)指揮，并與駕駛員互通情況及統(tǒng)一指揮信號，防止因信號不明確而造成誤操作。Adesignatedpersonshallbeappointedtoguideoperationofacrane.Thispersonshallcommunicatewiththecranedriversoastoconsolidateaunifiedoperationsignalstoavoidanymissoperatingincurredbywrongsignals;●起重機行走和停放處，應(yīng)注意保護(hù)地下電纜、管道、地溝或者其他保護(hù)措施。Thetrenchesandpowercablesandpipingembeddedundertheroadwhereahoistingmachineoracranewillgobyorstopataboveshallbewellprotectedwithnecessarymeasures;●吊車進(jìn)行吊裝時。應(yīng)伸足支腿，支腿下放置路基板。車身放置平穩(wěn)，在進(jìn)行起吊、行走、回轉(zhuǎn)等多項動作時，指揮人員應(yīng)視周圍情況，認(rèn)為安全后，方能指揮。Acraneshallgetitssupportswellstretchedontoabase-plateonthegroundbeforehoisting.Thecranemustpositionedstable.Thepersonwhowillguidetheoperationofthecarneshallcheckthesurroundingsandgetconfirmedthatthereisnothingunsafebeforethecranelifts,movesandrotates;●使用的吊索具應(yīng)有足夠的安全系數(shù)，并檢查鋼索的完好無損。Hoistingcablesshallbecheckedfortheirsafetycoefficientaswellastheircompletenessandingoodcondition.5.7.9 安全用火規(guī)定RulesforFireUsingonSite●氧氣、乙炔與油漆、油類物品須設(shè)置專用區(qū)，并分別存放，不用時蓋上瓶蓋，明確標(biāo)識。Oxygen,acetyleneandpaintandoilproductsshallbeclassifiedandseparatelystoredwithaclearmarking;●對有危險品如易燃/易爆場所，應(yīng)掛有“嚴(yán)禁煙火”警告牌，并配置相應(yīng)的消防、滅火器材。A“SmokeandFlameProhibited”signforwarningshallbeputatanareawherehazardousproductssuchasinflammables/explosivesarestored.Necessaryfire-fightingandextinguishersshallbearranged;●凡是用火的地方，應(yīng)按規(guī)定辦理動火證，制定防火措施，并設(shè)專人看護(hù)。工作完畢后，必須將火種徹底熄滅。A“WorkingRequestingFiringPermit”mustbeappliedforaworkneedsit.Necessaryprecautionsshallbetakenandaguardianforsupervisingshallalsobearrangedatthatareawhensuchworkisperforming.Allfiringsubstancemustbeclearedoutwhenthatworkiscompleted;●在高處動火（如電焊、氣焊、氣割）的下方周圍，如有易燃物應(yīng)予清除或采取用不燃物遮蓋，并設(shè)專人監(jiān)護(hù)。Whenaworkrequestingafiring(likeelectricwelding,gasweldingangascutting)isperformedatapositionabovetheground,anythingflammableunderitatthegroundshallbeclearedoutcompletelyorplaceanon-flammablesheetatsuchareabeforehand,aguardianforsupervisingshallalsobearrangedatthatarea;●撲滅火災(zāi)時，應(yīng)遵守電氣設(shè)施嚴(yán)禁用火和泡沫滅火的規(guī)定。油漆、油類、瀝青、有機溶劑等易燃液體及忌水化學(xué)物品，嚴(yán)禁用水滅火。Whenafireisunfortunatelycaused,therulesthatwaterandfoamsareprohibitedforextinguishelectricfacilitiesnorthewaterforpaint,oilproducts,asphalt,organicsolventsaswellaswater-forbiddenchemicals;●消防器材不得挪作它用，周圍不準(zhǔn)堆物，保持道路暢通。Fire-fightingequipmentordevicestobepositionedinaplacetowhichtheaccessisfreefromanyobstacleandcannotbeusedforanyotherpurposes.有關(guān)探傷工作的規(guī)定RulesforNDT探傷人員必須持有資格證書。ANon-destructivetestingtechnicianshallhaveaspecialpermitforhisjob;根據(jù)施工進(jìn)度做好射線探傷工作計劃，嚴(yán)格控制施工程序和施工范圍。Aworkplanshallbeworkedoutaccordingtoconstructionschedule.Theworkingareaandworkingproceduresshallbestrictlycontrolled;探傷區(qū)域應(yīng)有明顯標(biāo)志，并樹立“探傷危險”的警告牌，以防止其他人員進(jìn)入。Signsfwith“DangerforNDT”orwarningshallbeputatanareawherepeopleatthatareacaneasilyseetoavoidanyothersenterintowhenNDTiscarryingout;探傷工作至少要由兩人進(jìn)行，探傷工作人員應(yīng)有標(biāo)志。NDTshallbeundertakenbyatleasttwoNDTpersonswhoshallwearspecialjobmarks.5.7.11安全生產(chǎn)“六大”紀(jì)律：“SixDisciplines”forSafetyProduction進(jìn)入現(xiàn)場必須戴好安全帽，扣好安全帶，并正確使用個人勞保防護(hù)用品?！癢earingsafetyhelmet”,“fastenone’ssafetybelt”,“correctlyuseone’sprotectiveproducts”mustbewelldonebeforeonegoesintoasite;二米以上的高處，懸空作業(yè)，無安全設(shè)施的，必須戴好安全帶，扣好安全鉤。Onemustwearhelmetandfastenhissafetyhookwhenheisworkinginaplacewherethereisnosafetyprotectionsandis2metershighabovetheground;高處作業(yè)時，不準(zhǔn)往下或往上亂拋材料和工具等物體。Donottossorthrowdownanykindsofmaterialsandtoolswhenworkiscarryingoutatahighposition;各種電動機械設(shè)備，必須裝置漏電開關(guān)，有可靠有效的安全接地和防雷裝置，方能開動使用。Noelectricdrivenmachinecanstartworkingifitisnotequippedwithpowerleakageprotectiveswitchandreliableearthingaswellaslightningprotectiondevices;不懂電氣和機械的人員，嚴(yán)禁使用和玩弄機電設(shè)備。Anyoneotherthananelectricianisnotallowedtouseorplaywithaelectricdrivenmachineorfacility;吊裝區(qū)域非操作人員，嚴(yán)禁入內(nèi)，吊裝機械必須完好，扒桿垂直下方不準(zhǔn)站人。Anyoneotherthanapersonwhoisoperatingahoistingmachineoracraneisnotallowedtoenterintotheareawherehoistingiscarryingout.Hoistingmachinesmustbewellmaintainedandingoodcondition.Nooneispermittedtostandunderthecraneboom.5.8文明施工Civilizedconstruction文明施工是防止安全事故發(fā)生的重要組成部分，垃圾雜物和其它易燃材料堆積在施工現(xiàn)場，必然增加火災(zāi)的危險性和人員受傷的可能性。因此要求項目部管理人員和施工人員保證有一個清潔的工作環(huán)境，項目部管理人員應(yīng)定期進(jìn)行檢查，以保證文明施工條例的執(zhí)行。必須做到：Civilconstructionisaveryimportantparttoprotectincurringasafetyaccident.Garbageandotherinflammablematerialsthatarescatteredatsitewillincreasethepossibilityofincurringfireand/orcausingaccidentstoworkersatsite.Somanagepersonnelandconstructionworkersundertheprojectdepartmentshallpayattentiontothatandensuremakingtheconstructionsiteclean,theprojectmanagepersonnelmustcheckitatregularintervalsandmustensurethefollowing:5.8.1所用的材料、工具和設(shè)備適當(dāng)放置。過量的材料、工具和設(shè)備盡量不要放高處，擱置在高處的材料、工具和設(shè)備應(yīng)將其綁扎牢固，以防墜落傷人。Allmaterials,toolsandequipmentshallbeproperlyallocatedandplaced.Anyexcessofmaterials,toolsandequipmentshallnotbepositionedatahighplace.Thosealreadyplacedatahighplaceshallbefastenedfirmlytoavoidgettingthemdroppeddownandhurtpeopleconsequently; 隨時清理不用的材料和垃圾，并撤離現(xiàn)場。Anymaterialsthatarenolongerusedshallberemovedandanygarbageshallbeclearedout;5.8.3 滴漏的油料及溢出的流體應(yīng)立即清除，并經(jīng)合適的處理。Anyliquidthatisoverflowedshallbeimmediatelyremovedanddisposeit;5.8.4
