4.2.1負(fù)責(zé)工傷保險及工傷認(rèn)定和勞動能力鑒定。Workrelatedincidentinsurancecompensation,identificationofdegreeofinjuryandassessmentofinjuredsubject’sabilitytoworkfortheinjuredemployees.4.3各部門Individualdepartmentisresponsiblefor4.3.1負(fù)責(zé)事故的及時上報Reportingincidenttimely4.3.2內(nèi)部事故的現(xiàn)場初步處理。Preliminaryinvestigationatthesceneofincident4.3.3事故發(fā)生部門負(fù)責(zé)填寫“安全事故關(guān)閉報告”。DepartmentthattheincidenthasoccurredinisresponsibleforfillingoutIncidentClosureReport4.3.4支持和配合安全辦進(jìn)行工傷事故的調(diào)查、報告與處理；SupportandcooperatewithSafetyCouncilOfficetoconductworkrelatedaccidentinvestigation,reportandtreatmentissues.4.3.5Completingcorrectiveandpreventiveaction5,程序PROCEDURE5.1事故報告IncidentReporting5.1.1所有的事故包括未遂事故，財產(chǎn)損失事故，都必須上報安全辦。.Allincidentsnomatterhowminorincludingnearmiss,propertydamageshouldbereportedtoSafetyCouncilOffice.5.1.2在工作中員工所遭受的任何傷害，無論多么輕微，必須立即匯報給直接主管，并引起注意和進(jìn)行適當(dāng)?shù)闹委?。Anyinjuryhappensintheworkplace,regardlessofthedegreeofinjury,shouldbereportedtothedirectsupervisorimmediatelyandgivenappropriatemedicaltreatment.5.1.3事故發(fā)生后當(dāng)事人需立即向部門直接主管匯報，主管指導(dǎo)或現(xiàn)場協(xié)調(diào)事故的初步處理。Theinvolvedpartymustinformtohis/herdirectsupervisorimmediatelyaftertheincidenthappensandthesupervisorconductsorcoordinatesthepreliminaryprocessoftheincidentscene.5.1.4公司內(nèi)部事故發(fā)生后當(dāng)事人不具備匯報能力的，鄰近崗位對事故的匯報負(fù)責(zé)。Iftheinvolvedpartyhaslosttheabilitytoreporttheincidentaftertheincidenthappensinthecompany,thenearestemployeepresentshouldberesponsibleforreportingtheincident.5.1.5發(fā)生工傷事故時受傷員工或證人要立即聯(lián)絡(luò)部門主管，經(jīng)理，安全辦或醫(yī)生以便員工能得到及時的救治。具體的要求可參見急救管理辦法。Theinjuredemployeeorwitnessshouldcontactthedirectsupervisor,manager,Safetyofficerordoctortoseekfirstaid.Therelatedrequirementscanbefoundinfirstaidworkinstruction.5.1.6部門主管接到事故報告后立即報告安全辦.安全辦應(yīng)立即去事故現(xiàn)場開展調(diào)查。DepartmentsupervisorshouldreporttoSafetyCouncilOfficeimmediatelyafterreceivingtheincidentreporting。SafetyCouncilOfficeshallrushtotheincidentsceneforincidentinvestigation.5.1.7如果外來人員在我公司范圍內(nèi)發(fā)生傷亡事故，受傷人員，接待部門,目擊者或訪客要及時報告給安全辦Iftheemployeeofsupplierorcontractor,visitorwasinjuryintheplant,theinjuredperson,thereceptiondepartmentorthewitnessshouldreporttoSafetyCouncilOfficeandtakefirstaidmeasureormedicaltreatmentimmediately.5.1.8安全辦接到事故報告后在12小時內(nèi)通過郵件或電話通知HR經(jīng)理及公司高層。Afterreceivingtheincidentreporting,SafetyCouncilOfficeshouldreporttheincidenttoHRmanagerandtopmanagementwithin12hoursthroughe-mailorphone.5.1.9公司高層要在8小時內(nèi)將重大財產(chǎn)損失事故上報集團(tuán)安全健康總裁。MajorpropertydamageincidentshouldbereportedtoNewellRubbermaid’sCorporateDirectorofSafetyandHealthwithin8hoursaftertheincidentbytopmanagement5.1.10公司高層應(yīng)在1小時將死亡事故上報當(dāng)?shù)赜嘘P(guān)的政府部門及集團(tuán)安全健康總裁。FatalincidentshouldbereportedtolocalauthoritiesandNewellRubbermaid’sCorporateDirectorofSafetyandHealthwithin1houraftertheincidentbytopmanagement5.2事故調(diào)查Incidentinvestigation5.2.1所有的事故包括未遂事故，財產(chǎn)損失事故應(yīng)該被調(diào)查以防止再發(fā)生。Allworkrelatedincidentnomatterhowminorincludingnearmiss,propertydamageshouldbeinvestigatedtopreventtherecurrencethesimilarincident.5.2.2安全辦負(fù)責(zé)組織除死亡事故外的所有事故的調(diào)查，調(diào)查結(jié)果提交安全委員會討論改善。SafetyCouncilOfficewillorganizeateamtoinvestigateintoallincidentsexpectforfatalincident.InvestigationresultsshouldbesubmittedtoSafety&healthcommitteefordiscussionofthepreventiveandcorrectiveactions.5.2.3死亡事故上報后由公司所在地的安全生產(chǎn)監(jiān)督局組織調(diào)查。責(zé)任部門及安全辦應(yīng)配合安監(jiān)局的調(diào)查。ThelocalWorkSafetySupervisionBureauwillinvestigateafatalincidentaftertheincidentsarereported.TheresponsibledepartmentandSafetyCouncilOfficeshouldcooperatewithWorkSafetySupervisionBureauinincidentinvestigation.5.2.4對于嚴(yán)重的傷亡事故，社保局會自動到公司進(jìn)行現(xiàn)場調(diào)查，責(zé)任部門及安全辦應(yīng)配合政府的調(diào)查。Incaseofaseriousaccidentwithcasualty,SocialSecurityBureaumayconductanonsiteinvestigationautomatically.TheresponsibledepartmentandSafetyCouncilOfficeshouldcooperatewithSocialSecurityBureauinincidentinvestigation.5.2.5事故發(fā)生后，在保證事故當(dāng)事人得到救護(hù)的情況下保護(hù)事故現(xiàn)場直到調(diào)查結(jié)束。Aftertheincidenthadhappened,theincidentscenemustbesecuredtilltheon-sceneinvestigationiscompletedprovidedthatthepartygetsmedicaltreatmentfirst.SafetyCouncilOfficehaspowertosealtheincidentsceneifneed.5.2.6事故現(xiàn)場必須及時處理的，事故部門負(fù)責(zé)拍攝事故發(fā)生狀態(tài)照片，保存事故證據(jù)。Incidentsceneshouldbedealtwithtimelyandtheinvolveddepartmentisresponsiblefortakingphotosandsecuringtheevidence.5.2.7 Investigationteamshouldincludethefollowingmembersifpossiblea)公司高層Topmanagementb)安全主任SafetyOfficerc)部門經(jīng)理及工程部經(jīng)理DepartmentmanagerandEngineeringdepartmentmanagerd)設(shè)備操作、維修人員及工程師Facilityoperator，maintenancetechnicianorengineere)目擊者Witnessf)受傷員工Injuredemployee5.2.8調(diào)查結(jié)束后安全主任編制“事故調(diào)查報告”，提交公司高層。Aftertheinvestigationworkiscompleted,SafetyCouncilOfficewillcompiletheIncidentReportandsubmittotopmanagementforapproval.5.2.9安全事故調(diào)查報告應(yīng)于一周內(nèi)完成。Incidentinvestigationreport”shouldbefinishedwithinaweekafterincident.5.3.事故處理AccidentHandling：5.3.1.事故發(fā)生部門按照事故調(diào)查報告所提出糾正預(yù)防措施進(jìn)行整改Thedepartmentthataccidentoccurredisresponsibleforimplementingthepreventiveandcorrectivemeasuresputforwardbyincidentinvestigationteam.5.3.2..事故發(fā)生部門應(yīng)在事故處理完后的提交事故關(guān)閉報告，安全辦對落實的措施進(jìn)行驗證，確保措施的落實。TheconcerneddepartmentshouldsubmittheclosurereportfortheaccidenttoSafetyCouncilOffice.SafetyCouncilOfficeisresponsibleforidentifyingthepreventiveandcorrectivemeasuresandensuringthatthemeasuresfortheaccidenthasbeencompleted。5.3.3.對于任何有意謊報、不報或遲報工傷事故的，故意破壞事故現(xiàn)場，毀滅有關(guān)證據(jù)的，未及時消除安全隱患導(dǎo)致事故，違反操作規(guī)程，將追究責(zé)任人(包括受傷者本人)的責(zé)任，按公司紀(jì)律政策以相應(yīng)的處分,觸犯法律的，送交司法部門依法處理。Theresponsiblepersonwhoconcealtheincident,reportthefalseincidentonpurpose,delaytheincidentreport,ruintheincidentsceneonpurpose,destroytherelevantevidence,failtoeliminatethehiddendangertimelytoresultinincidentandisviolationofsafetyoperationrulemustbeartheresponsibilities.thepersonconcerned(includinginjuredemployee)shouldbepunishedaccordingtodisciplineofcompany.Ifbreakingcriminallaw,thepersonconcernedshouldbearcriminalliabilities.5.4.工傷保險及工傷認(rèn)定WorkRelatedInjuryInsurancesandWorkRelatedInjuryIdentification5.4.1HR應(yīng)及時向社保部門支付工傷保險費。所有在員工都應(yīng)參加工傷保險，包括臨時工HRdepartmentisresponsibleforpayingtheinsurancepremiumtolocalsocialsecuritybureauforallemployeesincludingtemporaryemployees.5.4.2發(fā)生的事故根據(jù)國家有關(guān)的法規(guī)屬于工傷的，，由HR在事故發(fā)生日之日起30日內(nèi)向社保部門申報工傷。Iftheincidentisconsideredasworkrelatedinjuryaccordingtoworkrelatedinjuryinsuranceordinance,HRdepartmentisresponsibleforapplicationforworkrelatedinjuryidentificationtolocalsocialsecuritybureauwithin30workdaysafteraccident5.4.3.經(jīng)治療后傷情相對穩(wěn)定后存在殘疾、影響勞動能力，需要進(jìn)行勞動能力鑒定的，由HR部門或員工本人向當(dāng)?shù)貏趧幽芰﹁b定委員會提出鑒定申請。Theinjuredemployeeisfullrecoveredafterreceivingmedicaltreatmentbuthis/herabilitytoworkmaybeaffectedifneed,HRdepartmentortheinjuredemployeemaybeapplicationforworkcapacityassessmenttolocalcommitteeinchargeofworkcapacityappraisal.5.4.4致殘工傷事故的殘廢等級由地方社保局評定。ThedisabilitycaseshallbeidentifiedandclassifiedbylocalSocialSecurityBureau5.4.5對于上報到社保局的工傷事故，通過社保局的最終鑒定后，將會從社?；鹬邪匆?guī)定補償有關(guān)的費用。IfaworkrelatedincidentthatisreportedtoSocialSecurityBureau,theSocialSecurityBureaushallcompensatetheinjuredemployeefortherel
