(12t gross) P

3、ersonnel suspected there Were too many 4-1/2” tubing in the basket and unloaded it, one bundle at a time. (Same way it was back loaded to the SF #2 offshore) Calculated total weight was 24.8t gross. Logistics immediately reported condition to offshore for investigation.Cause Analysis: Poor planning

4、and communication. Poor hazard awareness. Did not follow manifesting procedure.Corrective Action: Share near miss in whole field through September 8th, 2013. SWL of basket must be confirmed by lifting crew when back loading pieces. Facility materials man must calculate and record total weight of bas

10、erformed before any material is back-loaded for transportation back to shore or to other facilities: Ensure all lifting equipment is in good condition according to HSE MS Level II procedure of Rigging and Lifting. Making sure the load is centralized and secured. Baskets are not overfilled or overloa

11、ded, Ensure that all materials loaded into the containers are properly placed and they will not collapse or fall out when open the container doors. The tarpaulin should be in place to secure materials before closing the container doors if collapse or fall out risk foreseeable when loading. Record th

12、e cargos weight from cranes weight scale and indicate it on SAP cargo manifest.COPC HSE MS Procedure Material Handling When hazardous materials are returned to shore, make sure that the cargo is properly labeled and that the containers of hazardous materials are not leaking. “HEAVY LIFT” tags are ap

13、plied to the sling ring of equipment or material weight over Heavy Lift criterion. Ensure oily trash is proper handled with anti-pollution equipment and no leakage or spill; Boat captain has the right to reject any cargo if there is any potential risk on secondary containment. To ensure industrial w

14、aste, food waste, living waste, general waste is segregated and proper packed before back loading. Ensure separation of company material and contractor material Proper length of tag line is attached to the lift.
