1、PAGE PAGE 13醫(yī)療安全責(zé)任書為使全院醫(yī)務(wù)人員牢固樹立“以病人為中心”，救死扶傷，全心全意為人民服務(wù)的思想，強化質(zhì)量意識、醫(yī)療安全意識，職業(yè)風(fēng)險意識，消除醫(yī)療隱患，杜絕醫(yī)療差錯、事故”?！搬t(yī)療事故處理條例”及“執(zhí)業(yè)醫(yī)師法”等精神，制定醫(yī)療安全責(zé)任書如下: 一、各科應(yīng)建立健全以崗位責(zé)任制為中心的各項規(guī)章制度，認真執(zhí)行三級醫(yī)師查房制與手術(shù)分級管理制。按醫(yī)院優(yōu)質(zhì)服務(wù)、承諾服務(wù)的規(guī)定進行工作，做到按章辦事，違章必究。 二、各級各類醫(yī)務(wù)人員要注重醫(yī)德修養(yǎng)，認真履行崗位職責(zé)，堅守工作崗位，在崗時間不干私活，不從事醫(yī)療活動以外的任何事情。凡因違反而致事故、差錯、糾紛發(fā)生者，當(dāng)事人承擔(dān)全部責(zé)任。 三、

2、各級醫(yī)師的查房記錄或指示必須在病程記錄中明確無誤地顯示出來，上級醫(yī)師簽字認可;各種病歷書寫必須內(nèi)容真實完整，描述準確無誤，分析科學(xué)有序，記錄及時清楚，科主任要嚴格把關(guān)，不允許有重要缺陷的病歷歸檔?？剖矣嘘P(guān)人員及病案室管理人員要加強對現(xiàn)病歷及歸檔病歷的管理，防止丟失、遺漏、涂改、偽造、銷毀。不經(jīng)醫(yī)務(wù)科或主管院長批準，不得借調(diào)、摘抄、復(fù)印病歷。否則，引起糾紛，當(dāng)事人應(yīng)負全部責(zé)任。 四、嚴格執(zhí)行醫(yī)療技術(shù)操作規(guī)程，任何科室或個人不得隨意簡化或更改。一般診療技術(shù)操作前，要向病員及其家屬講明操作的目的、意義及操作中可能出現(xiàn)的反應(yīng)和副作用;對危重病員或在重要器官進行復(fù)雜的有創(chuàng)傷性的診療技術(shù)操作前，應(yīng)對患者或

10、與死亡病案討論，必須在院方規(guī)定的時間內(nèi)完成并出具書面記錄。對危重病人的病情變化或可能出現(xiàn)的并發(fā)癥要做到心中有數(shù)，要及時向病人家屬說明和記錄，必要時需病人家屬在病程記錄中簽字?？浦魅螒?yīng)定期檢查，組織實施。否則，因此而致事故、差錯、糾紛的發(fā)生，除追究當(dāng)事人的直接責(zé)任外，還要追究科室負責(zé)人的領(lǐng)導(dǎo)責(zé)任。 六、對一些特種檢查(包括CT、MRI等)、特殊治療(包括應(yīng)用貴重藥品等)醫(yī)療費用花費較高時，應(yīng)及時向病人家屬解釋清楚，取得家屬配合，減少醫(yī)療糾紛的發(fā)生。 七、凡重大手術(shù)和科室開展新業(yè)務(wù)、新技術(shù)、重要臟器切除等手術(shù)，必須按規(guī)定由科主任簽字后報醫(yī)務(wù)科、主管院長批準后方可實施;如施行緊急手術(shù)又無家屬及關(guān)系人

11、在場或其它特殊情況時，應(yīng)向科主任、醫(yī)務(wù)科或總值班報告。經(jīng)授權(quán)人批準后實施手術(shù)。否則，引起事故、差錯、糾紛，當(dāng)事人和科室應(yīng)承擔(dān)全部責(zé)任。 八、各科室應(yīng)對搶救設(shè)備、器械和藥品要做到專人保管、經(jīng)常檢查、及時維修，保證能隨時滿足搶救病人的需要。如因搶救器械、藥品不到位而引起事故、糾紛，當(dāng)事人和科室應(yīng)負全部責(zé)任。 九、新上崗人員不經(jīng)崗前培訓(xùn)不得進入科室?？剖覍π律蠉徣藛T應(yīng)抓好繼續(xù)學(xué)習(xí)、業(yè)務(wù)培訓(xùn)及管理工作，經(jīng)科室考核，確能單獨勝任工作時，由所在科室寫出申請，醫(yī)務(wù)處批準，方享受處方權(quán)單獨值班，遇有疑難問題時，應(yīng)及時報告上級醫(yī)師共同處理。否則，引起事故、a large area full Jet wide u

19、檢查，輸血同意書應(yīng)履行簽字手續(xù)。否則，引起事故、差錯及糾紛，當(dāng)事人應(yīng)負主要責(zé)任。 十一、科室必須加強對進修實習(xí)人員的管理，進修實習(xí)生書寫的各種醫(yī)療文件，帶教老師必須認真審查修改簽名認可或作補充記錄，其參加手術(shù)或進行各項診療操作，必須經(jīng)帶教老師同意并進行現(xiàn)場指導(dǎo);帶教老師不得隨意讓進修實習(xí)人員代替值班，若有違反引發(fā)差錯、事故、糾紛，帶教老師或值班醫(yī)生作為直接責(zé)任人首先予以追究。 十二、門診及病房首診醫(yī)師對接診病員的檢查、診斷、會診、處置、病歷書寫、轉(zhuǎn)科均負有責(zé)任。急危重病人到院后，接診醫(yī)師必須在5分鐘內(nèi)開始處置，并迅速報告上級醫(yī)師;危重病人需入院，轉(zhuǎn)送途中必須有醫(yī)護人員陪送，科室不得以任何理由拒

20、收病人。若有違反，引發(fā)差錯、事故，當(dāng)事人應(yīng)負全部責(zé)任。 十三、執(zhí)行保護性醫(yī)療制度，對某些嚴重或目前還不能醫(yī)治的疾病，應(yīng)對病員作嚴格保密，以減輕病員的思想負擔(dān)，防止意外事件發(fā)生。但對家屬、單位要如實反映病情及預(yù)后。對病員的隱私應(yīng)嚴格保密，不得隨意泄露;對其它科室或人員發(fā)生的糾紛，有關(guān)醫(yī)務(wù)人員不得將真象隨意、過早泄露給病人或家屬，以避免糾紛擴大、復(fù)雜化，否則將追究當(dāng)事人的責(zé)任。 a large area full Jet wide using insecticides on a regular basis for preventive measures and killing pests, to

27、four groups, namely, conservation of trees and flowering shrubs group 十四、全院各類工作人員要互相支持，團結(jié)協(xié)助，不相互拆臺，更不能在醫(yī)患之間拔弄是非，若借病員之口達報復(fù)個人之目的，制造醫(yī)療糾紛。否則，同此引發(fā)醫(yī)療糾紛者，當(dāng)事人應(yīng)參與糾紛處理的全過程，并視情節(jié)和后果，給予檢查、停職、行政處分等處理。 十五、加強對麻醉科的管理，對各科手術(shù)應(yīng)嚴格執(zhí)行麻醉操作規(guī)程和無菌制度，保證手術(shù)器械和設(shè)備的完好，發(fā)生故障及時維修，對違反麻醉操作規(guī)程而引發(fā)的事故、差錯、糾紛，追究當(dāng)事人的全部責(zé)任。 十六、認真落實院內(nèi)總值班和內(nèi)外科二線值班制度。

28、值班人員應(yīng)堅守工作崗位，隨叫隨到，積極參加或協(xié)調(diào)對病員的搶救、治療，對重大災(zāi)害、意外事件批量傷員和突發(fā)事件，必須及時逐級上報，不得拖延，否則引起事故、糾紛應(yīng)追究當(dāng)班人員的全部責(zé)任。 十七、醫(yī)、護、技人員要認真執(zhí)行各種查對制度，醫(yī)囑、處方、藥品、手術(shù)、輸血、收集標本以及簽發(fā)的各類報告等均按要求認真查對，以達到準確無誤，確保病人安全。護理人員要作好三查七對，嚴格執(zhí)行醫(yī)囑，經(jīng)常巡視病房，認真觀察病情，書寫護理記錄。及時、準確地向醫(yī)師反映病員的病情變化，對病員認真負責(zé)，否則引起事故、差錯、糾紛，當(dāng)事人應(yīng)負全部責(zé)任。 十八、糾紛一經(jīng)發(fā)生，當(dāng)事科室要妥善保留一份原始資料，如針管、殘存液體、血液制品等，病歷

43、oups, namely, conservation of trees and flowering shrubs group 2、各科室認真開展安全生產(chǎn)法制宣傳教育和培訓(xùn)活動。重點抓好安全生產(chǎn)法、江蘇省安全生產(chǎn)條例的宣傳及宣傳月活動。認真開展“百日安全生產(chǎn)活動”，在“百安活動”中無安全生產(chǎn)事故發(fā)生。 3、高度重視安全生產(chǎn)工作，把安全生產(chǎn)工作納入重要議事日程，每月參加院部安全生產(chǎn)工作例會，會議確定的各項防范措施必須嚴格實施。完成上級交辦所有安全生產(chǎn)有關(guān)工作。 4、各科室每季度對本科室安全生產(chǎn)進行一次大檢查，記錄齊全。事故隱患有登記，有整改措施，重大隱患及時上報院辦。 5、加強醫(yī)療質(zhì)量與醫(yī)療安全管

44、理，防范和減少醫(yī)療事故發(fā)生。 6、做好經(jīng)常性消防安全工作，做好易燃易爆物品安全管理工作，積極、主動、組織參與上級部門組織的專項整治活動。 7、加強科室管理，科室內(nèi)嚴禁私自使用生活用電器，違者將對使用人及科室負責(zé)人予以處理。落實好防盜措施，下班前關(guān)閉門窗，關(guān)閉機器設(shè)備、水、氣等開關(guān)，斷開電源。 8、對發(fā)生重大安全生產(chǎn)事故的科室實行一票否決，年度內(nèi)科室不得評先、評優(yōu)，安全生產(chǎn)的第一責(zé)任人和分管責(zé)任人不得評獎與提拔。 、嚴格落實安全生產(chǎn)信息上報制度。一旦發(fā)生安全生產(chǎn)事故，要迅速采取有效措施組9織搶險救援工作，防止事故擴大，最大限度地降低事故損失。同時還必須第一時間逐級上報，確保信息快捷、暢通。 單位
