2、gulati on.2. 范圍 /Scope適用于全公司范圍內(nèi)的危險(xiǎn)作業(yè)安全管理，包括動(dòng)火作業(yè)、高處作業(yè)。This procedure ruled all hazardous operati ons in clud ing hot work; work at height;3. 定義/Defi niti ons3.1 動(dòng)火作業(yè)：進(jìn)行焊接與切割作業(yè)及在易燃易爆場(chǎng)所使用噴燈、電鉆、砂輪等進(jìn)行可能產(chǎn)生火 焰、火花和赤熱表面的臨時(shí)性作業(yè)；Hot work : Any work involving welding, cutting, grinding or other similar operation

3、 producing sparks.3.2 高處作業(yè)：利用腳手架、升降平臺(tái)、便攜式木梯和金屬梯等登高工具從事超過(guò)基準(zhǔn)面2米以上，存在墜落風(fēng)險(xiǎn)的作業(yè)；Work at height :4. 程序 /Procedure序號(hào)NO.危險(xiǎn)作業(yè)內(nèi)容Hazardous operati on4.1動(dòng)火作業(yè)Hot work4.1.1職責(zé) Resp on sibility4.1.1.1申請(qǐng)人的職責(zé)（必須由我司項(xiàng)目負(fù)責(zé)人擔(dān)當(dāng)）：1. 動(dòng)火作業(yè)前負(fù)責(zé)動(dòng)火作業(yè)申請(qǐng)，并遵守一天一證一動(dòng)火點(diǎn)”為原則；2. 對(duì)外來(lái)施工單位的資格合法性審查，提醒外來(lái)施工單位帶齊動(dòng)火所需要的相關(guān)證件、防 護(hù)裝備；以備我司檢查；3. 對(duì)動(dòng)火作業(yè)現(xiàn)場(chǎng)

4、進(jìn)行確認(rèn)，確?，F(xiàn)場(chǎng)符合動(dòng)火要求；4. 對(duì)動(dòng)火人員的勞動(dòng)防護(hù)、動(dòng)火設(shè)備的安全狀態(tài)負(fù)責(zé)；5. 對(duì)動(dòng)火人員進(jìn)行培訓(xùn)，確保熟悉動(dòng)火作業(yè)要求、消防器材使用方法和應(yīng)急處理流程 要時(shí)，聯(lián)系EHS人員助指導(dǎo)）；6. 每天上、下午各不少于一次不定時(shí)對(duì)動(dòng)火現(xiàn)場(chǎng)進(jìn)行巡視，發(fā)現(xiàn)異常立即予以糾正；7. 所有供應(yīng)商外服人員、外來(lái)施工人員等進(jìn)行的焊接、氣割、烘烤等明火作業(yè)均需要提前 申請(qǐng)；Resp on sibility of applica nt （The applica nt should be the project leader）:1. 10、3動(dòng)火人職責(zé)1. 查驗(yàn)動(dòng)火作業(yè)申請(qǐng)單審批情況，無(wú)動(dòng)火作業(yè)申請(qǐng)單、申請(qǐng)單上相關(guān)流程未審批 完畢的應(yīng)拒絕進(jìn)行動(dòng)火作業(yè)；2. 逐項(xiàng)確認(rèn)相關(guān)安全措施的落實(shí)情況；3. 確認(rèn)動(dòng)火地點(diǎn)和時(shí)間，是否和動(dòng)火作業(yè)申請(qǐng)單一致；4. 發(fā)現(xiàn)不具備安全條件時(shí)不得進(jìn)行動(dòng)火作業(yè)；5. 隨身攜帶動(dòng)火作業(yè)申請(qǐng)單，以備檢查；6. 所有金屬焊接、氣割作業(yè)人員必須有金屬焊接切割作業(yè)操作證”，并熟悉動(dòng)火要求及消防器材使用方法；7. 動(dòng)火人員必須佩戴焊工專用絕緣手套、絕緣鞋及其它防護(hù)用品；8. 在動(dòng)火作業(yè)完成后，清理現(xiàn)場(chǎng)，清除殘火，確認(rèn)無(wú)遺留火種后方可離開(kāi)現(xiàn)場(chǎng)。Resp on sibility of fire operator:1. Clean the job site; make sure no fire in job site when finishing fire operation.4.1.1.4監(jiān)火人職責(zé)1. 堅(jiān)守崗位，不準(zhǔn)脫崗若暫時(shí)離開(kāi)需找適合人員替崗，并盡快返回；動(dòng)火期間，不準(zhǔn)兼做 其它工作；2. 負(fù)責(zé)現(xiàn)場(chǎng)動(dòng)火前的準(zhǔn)備工作并監(jiān)護(hù)與檢查，發(fā)現(xiàn)異常情況應(yīng)立即通知?jiǎng)踊鹑送Ｖ箘?dòng)火作業(yè)，及時(shí)聯(lián)系有關(guān)人員采取措施；3. 當(dāng)發(fā)現(xiàn)動(dòng)火人違章作業(yè)時(shí)應(yīng)立即制止；4. 在動(dòng)火作業(yè)完成后，應(yīng)會(huì)同有關(guān)人員

14、清理現(xiàn)場(chǎng)，清除殘火，確認(rèn)無(wú)遺留火種后方可離開(kāi)現(xiàn)場(chǎng)。5. 監(jiān)火人需在動(dòng)火作業(yè)完成1小時(shí)及3小時(shí)內(nèi)，返回現(xiàn)場(chǎng)檢查確認(rèn)無(wú)遺留火種。Resp on sibility of fire watcher:1. Fire watcher must constantly roam the work point that he is assigned for, aware for pote ntial hazards an d/or cha nging work con diti ons2. When there are some illegal operati ons, shut dow n hot work.4. Clea n the job site together with fire operator and make sure there is no fire ri

The fire watcher shall be presented for at 1 hour and 3 hours after hot work is done.4.1.2動(dòng)火作業(yè)安全要求Safety requireme nt for hot work:4.1.2.11. 所有動(dòng)火申請(qǐng)由本公司項(xiàng)目負(fù)責(zé)人負(fù)責(zé)，以一天一證一動(dòng)火點(diǎn)為原則，負(fù)責(zé)現(xiàn)場(chǎng)安全檢查和應(yīng)急處理，熟悉消防器材使用方法；2. 動(dòng)火人、監(jiān)火人都必須熟悉動(dòng)火要求，并承擔(dān)現(xiàn)場(chǎng)監(jiān)督、防火管理責(zé)任。3. 五級(jí)以上風(fēng)力或雨雪天氣，禁止室外動(dòng)火作業(yè)；4. 所有金屬焊接、切割作業(yè)人員必須有金屬焊接切割作業(yè)操作證”為防止觸電事故的

17、發(fā)生，動(dòng)火人員必須佩戴焊工專用絕緣手套、絕緣鞋及其它防護(hù)用品，包括外服人員和外來(lái)施工人員，并保證焊機(jī)絕緣層無(wú)破損；5. 動(dòng)火作業(yè)前必須將周圍易燃易爆物品移至安全位置。一般情況下，在施工區(qū)10m范圍內(nèi)，無(wú)可燃物，易燃液體，易燃?xì)怏w，可燃粉塵等均清理干凈。如仍不能滿足安全要求，若半徑10m范圍內(nèi)的易燃、易爆品不能撤離，用防火布或濕木板隔離；（特別說(shuō)明：靠近叉車電瓶區(qū)域作業(yè)時(shí)，必須將電瓶拆除或者切斷電瓶電源，并進(jìn)行充分通風(fēng)，保證作業(yè) 區(qū)域內(nèi)無(wú)燃爆氣體后，方可動(dòng)火，防止氫氣爆炸）MFG-S1-468Revisio n: 0Effective date: 07/17/2015PRINTED COPIES

19、少于 5米；氧氣瓶和乙炔氣瓶距離不少于5米。乙炔瓶禁止使用其它氣瓶進(jìn)行替代，并保證有止回火閥。所有氣瓶必須無(wú)破損，壓力正常；8. 動(dòng)火現(xiàn)場(chǎng)配置兩只以上滅火器，必要時(shí)將消防水帶鋪設(shè)到作業(yè)現(xiàn)場(chǎng)。無(wú)火警時(shí)禁止使 用，否則照價(jià)賠償，使用完成，及時(shí)放回指定位置；9. 動(dòng)火結(jié)束后，動(dòng)火人員和監(jiān)火人須共同進(jìn)行確認(rèn)，保證現(xiàn)場(chǎng)無(wú)火警后，方可離開(kāi)作業(yè)現(xiàn) 場(chǎng)。動(dòng)火作業(yè)申請(qǐng)單應(yīng)交回 EHS留存。1. Fire watcher and operator must be familiar with safety requireme nt for hot work.3. Hot work in ope n air is forbidde n whe n there is bad weather such as

21、rain storm, and heavy wind above five grades that in flue nee the safety of work.4. Fire operator must be trained specially, and gets the license. They should wear proper pers onal protective equipme nt to en sure safety operati on. Any damaged or frayed cord in weldi ng mach ine should remove from

22、service. The con structors are required to follow this requireme nt.5. Exposed flammable/combustible materials and equipme nt with a 10m radium must remove or be protected by fire-proof covers or fire bla nkets before hot workbeg in s.(Remark: Shut off the power and keep good ven tilati on while fir

23、e operati on being done n ear charg ing area, it will preve nt gas explosi on)6. All combustible dust ,vapor, gas of liquids must be kept 10m away or properly covered with fire-proof curtains, fire blankets. It means weld/cut within 10m was forbidde n while a paint booth or shot blaster was running.

24、7. Oxyge n cyli nder should keep at least 5m away from acetyle ne cyli nder. Mea nwhile gas cyli nder should keep 5m away from ign iti on source and in good con diti on.8. 33、 /Hazardous operation management procedureoperati on and take corrective acti ons.4.2.1.4作業(yè)人員的職責(zé)1. 針對(duì)身體狀況負(fù)責(zé)，有咼處作業(yè)禁忌的癥狀（咼血壓、心臟病、貧血、癲癇等）視力不 佳、疲勞過(guò)度等不適宜進(jìn)行高處作業(yè)的情況，嚴(yán)禁高處作業(yè)；2. 查驗(yàn)高處作業(yè)申請(qǐng)單的審批情況，無(wú)高處作業(yè)申請(qǐng)單、申請(qǐng)單上相關(guān)流程未審 批完畢的應(yīng)禁止作業(yè)；3. 掌握高處作業(yè)的安全要求，并逐項(xiàng)確認(rèn)相關(guān)安全措施的落實(shí)情況；4. 配齊防護(hù)設(shè)備，對(duì)于危險(xiǎn)性過(guò)高且無(wú)法進(jìn)行安全保障的應(yīng)禁止作業(yè)；5. 高于5米的高處作業(yè)（3個(gè)腳手架高度）必

A regular physical for operators en gaged in work at height is required: people who has occupati onal con tra in dicati on （such as hyperte nsion, cardiopathy, an emia, epilepsy, and men tal ill ness）, old ness, weak ness, overtired,

35、 poor eyesight or other improper people are not allowed to engage in work at height.2. Check out the work has got approved. Operator has right to refuse perform the work if there is no work permit.3. Un dersta nd safety requireme nt for work at height, and check out preve ntive measures one by one a

36、ccord ing to work permit.4. Wear proper pers onal protective equipme nt, operator has right to refuse the work with high risk.5. If operator is work ing above 5m, he shall receive special tech no logy training, pass releva nt test and gain authorizati on.6. Check out the job site and work time is ac

39、度)必須持證上崗；3. 高處作業(yè)必須佩戴好安全帽、安全帶；安全帽必須扣緊下顎帶，鎖緊后定鎖；安全帶必 須系在作業(yè)處上方的牢固構(gòu)件上或?qū)閽彀踩珟У匿摷芑蜾摻z繩上，不得系掛在移動(dòng)或 不牢固的物件上；不得系掛在尖銳棱角部位，不得底掛高用；4. 高處作業(yè)監(jiān)護(hù)人必須全程監(jiān)護(hù)，堅(jiān)守崗位；對(duì)使用靠墻梯進(jìn)行高處作業(yè)的，監(jiān)護(hù)人員必 須扶住梯梁；5. 高處作業(yè)所使用的工具、材料、零件必須裝入工具袋；6. 高處作業(yè)時(shí)，工具、材料、零件嚴(yán)禁隨意亂置或向下丟棄，使用完畢后，必須及時(shí)放入 工具袋中.7. 雨天、雪天、6級(jí)以上強(qiáng)風(fēng)、濃霧等惡劣氣候，嚴(yán)禁室外登高作業(yè)；8. 夜間登高作業(yè)，必須有足夠的照明；Safety re
