8、外保險。乙方獲得當(dāng)?shù)毓矙C(jī)關(guān)頒法的居留許可證后 的下一個月起，甲方為乙方購買社會保險 （ 養(yǎng)老保險、綜合醫(yī)療保險、住院醫(yī)療保 險和工傷保險 ） 。社會保險卡將由政府職能部門于乙方取得居留許可證后 50 個工作日左右發(fā)放給乙方 （ 節(jié)假日順延 ） 并開始持卡享受中國國民一樣的上述社會 保險待遇。乙方如需就醫(yī)，須在社會保險定點醫(yī)院就醫(yī)并按社會保險規(guī)定承擔(dān)相 應(yīng)比例的費用，在非定點醫(yī)院就醫(yī)費用自理 ; 受聘方的掛號、就醫(yī)交通、鑲牙、整 容、保健按摩、配眼鏡、住院就餐和服用非醫(yī)療性的滋補(bǔ)藥品以及醫(yī)生出診等費 用自理。Insurance :party A will buy personal accide

9、nt and public transportation risks insurance for party B after both arriving in China on a work Z.visa and signing a formal employment contract.party A will buy social insurance （ including pension,comprehensive Medicare,hospitalization and injury on job site ） for party B will after getting the Res

10、ident permit from the public Security Bureau.Under this insurance card will be issued by the Social Insurance Bureau around 50 work days after receiving the Resident permit(will be delayed if theres a holiday). If party B needs to see a doctor ,he/she has to visit doctors at designated hospitals cov

11、ered by the social insurance and undertake the relevant proportion of expenses.If party B sees a doctors at designated hospitals covered by the social insurance and undertake the relevant proportion of expenses.If party B sees a doctor at a hospital not covered by the insurance plan,he/she needs to

12、pay all the expenses; including outpatient registry,transportation,dentalcare,face-lifting,therapeuticmassage,filling a prescription forglasses,hospitalized dinning and taking of nutritious food as well as appointing doctors.五、其他條款以報到后簽訂的正式合同為準(zhǔn)。For other conditions of employment piease refer to the

13、formal contact which will be signed after arrival.六、注意事項：Notice:1. 乙方在簽署聘用意向書后應(yīng)該立即準(zhǔn)備以下文件給甲方用于辦理簽證相關(guān) 文件：party B shall provide following documents after signing the Employment Intention Letter:1) 護(hù)照復(fù)印件及過往所有的中國簽證復(fù)印件Scanned copy of their passport and all Chinese visas2) 最高學(xué)歷證書掃描件Scanned copy of their hi

14、ghest school certificates.3) 教師資格證書掃描件Scanned copy of their teaching certificate4) 健康體檢報告掃描件Scanned copy of their healthy check5) 無犯罪記錄Scanned copy of their Non- criminal record6) 親筆簽署姓名的簡歷Scanned copy of their resume with signature at the bottom7) 推薦信Scanned copy of recommendation letters 乙方需在簽署意向書

15、后 1 個月內(nèi)提供甲方所需的辦證文件，乙方如因提供的 資料不齊或延誤，造成無法如期辦證，責(zé)任自負(fù)。All Documents requested must be provided by party B s visa2. 待甲方收集齊乙方提供的所需文件后，辦證時間需要 2 個月左右。相關(guān)文 件辦理完畢后甲方會將文件以快遞形式寄送給乙方，請乙方務(wù)必及時到所在地中 國使領(lǐng)館辦理工作簽證并持工作簽證入境。It will take about two months to process the visa application in China and only be started after par

17、保帶上最高學(xué)歷證書原件、教師資格證原件以及無犯 罪記錄原件以通過政府職能部門的審核。若無法提供原件或遭政府職能部門拒絕， 甲方將無法為乙方進(jìn)行居留許可申請并意味著聘用關(guān)系的提前終止。同時，乙方 入境后需進(jìn)行健康檢查和警察局面談，如因身體原因或在中國境內(nèi)曾有非法工作 / 犯罪行為等因素?zé)o法辦理居留許可的，甲方唯有解除合約。While coming to China to begin employment, party B must bring their original highest university diploma, original teaching certificate and

18、original police background check as requested by the Foreign Expert Bureau.Failure to bring these original documents or rejection by Government Bureaus for any reason will prevent party A from being able to apply resident permit for party B and willresult in early termination of the employment relationship. party B will need to take a healthy check and a police interview after entering China, if any factors to the fault of party B such as health or employment/legal history cause them fail to get the resident permit,p
