1、運(yùn)動(dòng)員參賽保證書 - 保證書本人對(duì)參加此次賽做以下保證：1 、 我保證：遵守比賽組委會(huì)所有規(guī)則和服從一切裁決。2 、 我保證對(duì)自己的參賽器材、 裝備及個(gè)人所有物品負(fù)有全部責(zé) 任。3 、我對(duì)在連續(xù)而激烈的比賽中可能發(fā)生的危險(xiǎn)已有充分的認(rèn)識(shí)， 我保證本人的身體健康狀況能夠承受本次比賽， 并具備近期健康檢查 證明書。4 、我保證：本人在比賽中因受傷、遭遇意外事故或發(fā)生疾病時(shí)， 對(duì)受到的急救處理方法及結(jié)果不提出異議。5 、我本人、我的家屬、親戚和保護(hù)人已充分理解本次比賽的內(nèi) 容和意義， 并認(rèn)可我本人參賽。 在比賽之前我已作了人身保險(xiǎn)。 因此， 我保證：我自己、親屬及遺囑執(zhí)行人、財(cái)產(chǎn)繼承人，對(duì)本人在比賽

2、中 或參加由本次比賽委會(huì)組織的與比賽有關(guān)的活動(dòng)時(shí)， 不論什么原因所 致的傷、病、亡或出現(xiàn)的后遺癥等，不對(duì)比賽組委會(huì)提出任何起訴。 所造成的損失由保險(xiǎn)公司和本人承擔(dān)， 比賽組委會(huì)不再提供因此而造 成的損失費(fèi)。6 、我保證：允許比賽組織者自由使用和制作與比賽有關(guān)的我的 名字、照片等。并承認(rèn)與本次比賽有關(guān)的新聞、廣播、電視等商業(yè)宣 傳報(bào)道。7 、我保證：由于自然災(zāi)害等客觀原因造成比賽取消或變更比賽 時(shí)間、內(nèi)容所引起的有關(guān)費(fèi)用問題，服從比賽組委會(huì)的決定；由于我 個(gè)人的情況而不能參加比賽的，不要求退還參賽時(shí)所交納的各種費(fèi) 用。8 、我確認(rèn)：對(duì)參賽所進(jìn)行的上述保證已充分理解，并承認(rèn)本保 證書解釋權(quán)屬比賽組
