Neoasphyxia:NewbornsLearningEnglishCourseware目錄IntroductiontoNeoasphyxiaTheImportanceofNewbornsLearningEnglishTheimpactofNeoasphyxiaonnewbornslearningEnglishPreventionandtreatmentofNeoasphyxia目錄SuggestionsfordesigningcoursewareforlearningEnglishfornewbornsCasesharingandsummary01IntroductiontoNeoasphyxiaNeoasphyxia,新生兒窒息,是指由于產(chǎn)前、產(chǎn)時(shí)或產(chǎn)后的各種原因?qū)е绿喝毖醵l(fā)生宮內(nèi)窘迫或娩出過程中發(fā)生呼吸、循環(huán)障礙,導(dǎo)致生后1分鐘內(nèi)無自主呼吸或未能建立規(guī)律呼吸,以低氧血癥、高碳酸血癥和酸中毒為主要病理生理改變的疾病。定義新生兒窒息具有起病急、進(jìn)展快、病情重的特點(diǎn),若不及時(shí)處理,可導(dǎo)致嚴(yán)重的腦損傷、多臟器功能衰竭、甚至死亡。特點(diǎn)DefinitionandCharacteristics病因新生兒窒息的常見原因包括母體因素、胎盤因素、胎兒因素和分娩因素等。其中,母體因素包括妊娠期高血壓、糖尿病等;胎盤因素包括前置胎盤、胎盤早剝等;胎兒因素包括宮內(nèi)發(fā)育遲緩、早產(chǎn)兒等;分娩因素包括產(chǎn)程延長(zhǎng)、胎位異常等。發(fā)病機(jī)制新生兒窒息的發(fā)病機(jī)制主要與缺氧和酸中毒有關(guān)。缺氧可導(dǎo)致細(xì)胞能量代謝障礙,引起細(xì)胞內(nèi)ATP生成減少,細(xì)胞膜通透性增加,導(dǎo)致細(xì)胞內(nèi)水分和鈉離子潴留,引起細(xì)胞水腫和顱內(nèi)高壓。酸中毒則可抑制中樞神經(jīng)系統(tǒng)和呼吸系統(tǒng),加重缺氧,引起呼吸衰竭和循環(huán)衰竭。Etiologyandpathophysiology新生兒窒息的診斷主要依據(jù)生后1分鐘內(nèi)的Apgar評(píng)分。Apgar評(píng)分包括皮膚顏色、心率、呼吸、肌張力和喉反射等5個(gè)方面,每項(xiàng)0-2分,總分0-10分。正常為8-10分,4-7分為輕度窒息,0-3分為重度窒息。診斷標(biāo)準(zhǔn)新生兒窒息需要與其他引起生后低氧血癥和呼吸困難的疾病進(jìn)行鑒別,如新生兒肺炎、先天性心臟病等。此外,還需要與顱內(nèi)出血、中樞神經(jīng)系統(tǒng)感染等引起驚厥、昏迷等癥狀的疾病進(jìn)行鑒別。鑒別診斷Diagnosticcriteriaanddifferentialdiagnosis02TheImportanceofNewbornsLearningEnglish
